I hereby certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for my child(ren) to participate in the West University Place Piranhas Swim Team. In the event that my child(ren) should require medical attention, I hereby authorize the City of West University Place and/or the West University Aquatic Club, Inc., d/b/a the West University Piranhas Swim Team to take whatever action is deemed necessary in their judgment. This includes the authority to seek medical assistance for my child(ren) and to sign releases on my behalf for medical services for my child(ren). I agree to be responsible for any and all costs associated with any necessary medical attention and/or treatment.

On behalf of myself, my spouse,and my child(ren), I hereby release, acquit, and discharge the City of West University Place and/or the West University Aquatic Club, Inc., d/b/a the West University Piranhas Swim Team and said entities’ past, present, and future officers, agents, servants, representatives, employees, coaches, assistant coaches, volunteers, insurers, sponsors, associates, and related organizations who may be liable in any way or manner whatsoever from from any and all claims, losses, demands, actions, causes of action, suits and rights, known or unknown, past, present or future, foreseen or unforeseen, asserted or unasserted, accrued or unaccrued, including but not limited to all physical and mental injuries, death, pecuniary losses, property damages, and all damages of whatever nature arising out of or relating to my child(ren)‘s participation in swim team practices, meets, or other team events. I further assume all risks and hazards incidental to my child(ren)’s participation in the swim team for which I am enrolling. This release also extends to any firm publishing and/or distributing photographs of my child(ren), whether on paper or via electronic media, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.

I further agree to indemnify and hold harmless the parties hereby released, REGARDLESS OF THE NEGLIGENCE IN WHOLE OR IN PART OF THE PARTIES NAMED ABOVE, from all claims, demands, actions, and causes of action brought by anyone acting on my behalf or on behalf of my child(ren) for any claim arising out of, resulting from, or in any manner connected with or relating to the my child(ren)’s participation in swim team practices, meets, or other team events.

COVID-19. COVID-19 is an extremely contagious disease that can lead to severe illness and death. While the City of West University Place and/or the West University Aquatic Club, Inc., d/b/a the West University Piranhas Swim Team have undertaken enhanced health and safety measures to protect the public, an inherent risk of exposure exists in any public place where people are present. By participating in the activity, event or use I am signing up for, I voluntarily assume all risks related to exposure to COVID-19.
